Whistleblowers are entitled to submit their notifications through the reporting channels listed below:

a) electronically to a special e-mail address for receiving notifications

ipc.whistleblowing@seznam.cz

(hereinafter referred to as the “e-mail address”); only the competent person has access to the e-mail. This secure internet communication platform was established by the employer.

b) or orally, by arrangement with the competent person at the HR department at an agreed time on working days from Monday to Friday from 6:00 a.m. to 2:15 p.m. in office no. 2.03. A record will be made of the oral notification, with the subsequent possibility of checking, modifying, and approving the transcript of the oral notification by signing it.

c) notifications can be submitted in writing to the address I P C plast spol. s r.o., Company ID: 00515566, with registered office at Palackého 753, 394 68 Žirovnice. The notification must be submitted in a properly sealed envelope marked “WHISTLEBLOWING – TO THE ATTENTION OF THE COMPETENT PERSON – DO NOT OPEN. Failure to comply with this condition may compromise the confidentiality of the whistleblower’s identity or the confidentiality of the information disclosed.

d) notifications can also be submitted to the employer into the box located at the HR officer’s and the competent person’s office, always on working days from Monday to Friday from 6:00 a.m. to 2:15 p.m. at office no. 2.03 in the HR department. The notification must be submitted in a properly sealed envelope marked “WHISTLEBLOWING – TO THE ATTENTION OF THE COMPETENT PERSON – DO NOT OPEN. Failure to comply with this condition may compromise the confidentiality of the whistleblower’s identity or the confidentiality of the information disclosed.

e) notifications can also be submitted to an external resolver, the Ministry of Justice of the Czech Republic, in writing: Ministry of Justice of the Czech Republic, Vyšehradská 16, Prague 2, Postal Code 128 10, or by e-mail: oznamovatel@msp.justice.cz or by telephone 00420 221 997 840.

The obligated entity – employer exercises its right and excludes the acceptance of notifications from a person who does not perform work or other similar activity for the obligated entity – employer under the provisions of § 2 paragraph 3 letters a), b), h) or i).
